BOTB Voting Slips

Who would like to design some BOTB voting slips for me Need to be black writing (need 6 slips to a A4 sheet)

The details which need to be on it is;

Can it not just be..

Just gives more room for people to write next to it on my slip.
If not, I'll find some other way around it.

Ok, I did it in Word, so I could make sure I could get 6 on an A4 sheet, and on that doc, they all have a border. But I did it also on photoshop so I could post it as an image on here to show you an example.



The only differences in the word doc are, as I said, the slips have a border, and "Last band playing" and "1st band playing" are in bold. For some reason, photoshop wouldn't let me make them bold.

Is it ok? I guess I would have to email the actual word doc though.

would it not be easier to number the bands? and put them in order of playing. eg shychlo first????
